If I am ever able to manage my health needs independently, am I obligated to continue receiving home health care?
No. Salus Homecare of Salt Lake City seeks your doctor’s advice about beginning and continuing services. If our services are no longer medically indicated or you choose to stop them, there is no contract that stipulates you must remain our client. Additionally, we never charge a change or cancellation fee when services are reduced or discontinued.
I would like to have one nurse who I can form a bond with. Is this something your agency can provide me with?
Having one home health nurse is possible at times, but it will depend on your medical needs and scheduling needs. Salus Homecare of Salt Lake City will ensure that you always have coverage, and we will always do our best to maintain consistent care so you are able to form relationship with your entire home health team.
Can you provide me some more details about a care plan?
A care plan is the step by step instructions that your home health nurses and therapists will follow when providing care. It provides them with information about your goals, what areas you need assistance in and how often care will be provided. It is developed with your input as well as input from your medical team, family members and a Salus Homecare of Salt Lake City care manager. The care plan can always be revised as your needs change and is constantly monitored to ensure it remains appropriate.
