Her kindness and compassion as well as the way she kept us informed about what was happening was so helpful. our bath aid was a great and she was so kind to mom. The plan of care is a document created during the assessment process and re-evaluated regularly. It is thorough and includes your goals and needs and specific information about the services and steps that will help you to meet them. Salus Home Health takes an interdisciplinary approach to developing the plan of care. Our input, your input, that of your family members and the medical professionals involved in your diagnosis, care and hospital discharge are all essential in ensuring it is accurate, thorough and appropriate.
Salus works hard to ensure that your long term home health care is consistently provided by the same professional care team, but in some cases, you may see more than one professional. Along with your nurse and case manager, social workers and therapists may assist in providing you with services and support. This will all be explained to you, and we will coordinate with you to ensure that, whenever possible, visits are conducted at times that are convenient for you and your family.
