To remain safe and comfortable in your own home or place of choice in times of recuperation or during the normal aging process

Assistance with activities of daily living can include bathing, grooming, meal preparation, and medication reminders. Also light housekeeping and other requested services.

Private pay; for those that have long term care insurance their policy may pay a good portion of the costs; for those qualifying war-time veterans (or spouse) the VA covers a small portion.

Home Health

Our home health services provide you with nursing care and therapeutic care, in the comfort of home. To regain prior level of function/health. Promote the patient's ability to move, reduce pain, restore function and prevent disability.

Multidisciplinary team approach. Patient's physician arranges Medical Equipment and Medications; must be authorized by insurance.

Not taking your medications as prescribed could lead to your disease getting worse. Additionally, it increases your risks of unpleasant and dangerous side effects as well as the potential for increased or ill-managed pain, rehospitalizations, slips and falls and even death. For seniors, mismanagement of medications increases the risk of hospital admissions or readmissions and reduces the likelihood of long term independence and aging in place. While many patients benefit from home health services when it comes to managing new or temporary medications appropriately, there are other strategies that can also work. They include asking a professional caregiver, friend or loved one to help with medication reminders, properly labeling your pill bottles and using a pill dispenser, and taking advantage of apps or phone reminders to remember to take medications on time. A home health nurse can help you to implement a strategy that works long term as part of your plan of care. While a Salus home health nurse can administer medications, this is not necessarily how medication management is best handled. Some patients prefer to receive instruction on how to take all their medications and assistance in filling pill boxes or preparing daily doses. Some patients also begin with their nurse administering medications and later transition to a more independent system. You will be involved in creating the right plan of care for your specific needs. An in home aide cannot administer medications. In home care is an appropriate service for a senior who simply needs medication reminders but are still capable of self-administering all of their medications. Medication administration is a skilled service and therefore, handled under in home health care. Yes, in some cases Medicare will cover medication management services. This is typically true when they are classified as a “skilled need” by your physician and necessary on a more short term basis. Our case managers can assist you in determining if this is a covered expense based on your doctor’s orders and specific needs. Yes, our nursing staff is fully trained to provide support with infused medications. We will simply need a doctor’s orders for the medication, and then it can be added to your plan of care. We will provide support in administering the medication, monitoring doses and providing information to your physician that can be helpful in determining any need for changes to your dosage. For most patients, medication management is part of a coordinated and thorough plan of care that addresses all skilled home health and non-skilled home care needs. Therefore, it would be the exception to the rule for a client to receive services strictly to help them better manage their medications.
