Salus’ palliative care team provides information and guidance to help you feel confident in the choices you make regarding advanced care planning. This key pillar to our palliative care program provides support to both the patient and family members so that you can better prepare for the unexpected and ensure your wishes are understood and respected.
Advanced care planning is a gift for your loved ones as much as it’s an important experience for you. Helping them to better understand your wishes and goals reduces stress, anxiety and uncertainty. It also empowers them to help you maintain control over your treatments in a dignified and respectful way.
Christiane S. Riverside January 16, 2026As a niece who lives in the Mid-West/ Milwaukee, I could not be directly involved in my Aunt’s end of life care. I flew out to California in October 2025 and arranged 24 hour home care for her when she was ready to be released from the hospital. Because of her age (98), suffering pain and being weakened through 4 weeks of hospital stay and rehab. Hospice care was recommended by her medical team. Salus was recommended by her Home care provider. Always the best care and was also on the hospital list of approved providers. After leaving California, I felt like my Aunt was in very good hands and back home where her friends and neighbors could easily visit.
William B. Riverside January 9, 2026Salus was very professional, caring and supporting of both my wife and me. I cannot imagine anyone else providing better care. Thank you Salus from the bottom of my heart.

Salus Palliative Care social workers are here to help you clearly communicate with your loved ones. They’ll coach you through this process, help you to answer important questions for yourself before having conversations with others and support you in staying true to your wishes. Social workers can also mediate during conversations and ensure that everyone clearly understands the information you present.
You are never required to have specific documents to take advantage of palliative care services. However, there are important reasons why we highly recommend these documents. They hold a lot of value in helping you communicate your wishes. We are happy to explain the importance of these documents and help you access community resources that can help with preparing them.
These decisions are yours to make. They are deeply personal and based on your values and beliefs. Our team is here to discuss your options and help you understand your choices, but your choices are final, and we will always respect them.
