To refer a client to Salus, simply call us at (888) 725-8742, and we’ll schedule an assessment. Alternately, fill out our referral request form, and a member of our team will get back to you within 24-hours. Your client’s journey with Salus will begin with a thorough assessment to better understand their needs and develop a plan of care that addresses those needs, as well as your client’s goals. Afterwards, a case manager will continue to oversee your client’s case, ensuring that services remain appropriate and beneficial, advocating on behalf of your client, and providing you with timely updates on their care, progress and setbacks. Our case managers remain available should you have any questions, and you will be provided with their cell phone number in order to ensure timely communication and response. This is a difficult question to answer without assessing the client as every situation is unique. However, if your client is losing weight, having a difficult time maintaining personal cleanliness or a clean home, falling (especially if there are repeated falls), struggling to manage day to day tasks, or if they are struggling with a chronic health condition or a life-limiting diagnosis, contact us. We can help determine what their needs are and provide the support they need.
