For most chronic illnesses, even the most well-intentioned family caregivers lack the knowledge and experience necessary to provide medical support for their loved one’s condition. With the assistance of Salus Home Healthcare, medical needs are properly addressed, families feel empowered to support their loved ones at home and individuals remain comfortable at home, reducing the need for placement or repeat hospitalizations.

Our caregiver was wonderful, helpful, and couldn’t have been more helpful in all needs that could have been provided!

My 104 year old mom was on both Salus Home Health and Salus Hospice, and the care she received was extraordinary. I have dealt with a number of healthcare companies in the two and a half years of my mom’s declining health, and Salus was by far the best. The Home Health nurses were very caring and compassionate, and they called before and after every visit with my mom. I could reach them on weekends, and at first I was disappointed to learn that they would not be the same nurses when my mom transitioned to Hospice. Well it turns out the Salus Hospice nurses were just as caring, always available, and took wonderful care of my mom up until the minute she passed. That can not be an easy job, and I have nothing but praise for everyone at Salus. Thanks for your care and help.

The plan of care is created during the assessment process and re-evaluated on a regular basis. It includes your goals and needs and specific information about the services and steps that will help you to meet them. Salus Homecare takes an interdisciplinary approach to developing the plan of care and includes input from you, your family members and the medical professionals involved in your diagnosis, care and hospital discharge.
The answer to this can depend on your specific health concern and how many hours a day or week you need services. While we typically try to keep home care nurses consistent, sometimes an illness necessitates different specialists, and this might mean you receive care from more than one person.
