Salus’ palliative care team in South Bay provides information and guidance to help you feel confident in the choices you make regarding advanced care planning. This key pillar to our palliative care program provides support to both the patient and family as they prepare for the unexpected. The most important goal is to ensure your wishes are understood and respected.

Advanced care planning is invaluable to your loved ones as much as it’s essential for you. Helping them to better understand your wishes and goals reduces stress, anxiety and uncertainty should you reach a point where you can’t fully communicate wishes and goals. It also helps ensure that you maintain control and direction over your treatments in a dignified and respectful way.

On behalf of my family, we would like to thank you all for your hard work and dedication, putting your amazing skills to work in handling end-of-life care for our dearly loved father, Curtis Holmes. Our father passed away Tuesday morning, 10/3/23, peacefully in our home. All I could think was how grateful my family and I are to have all of you and your team with us until the end. Salus Palliative Care professionals are here to help you clearly communicate with your loved ones. Our social workers will guide you through this process, help you to answer important questions for yourself before having conversations with others and support you in staying true to your wishes. Social workers can also mediate during conversations and ensure that everyone clearly understands the information you present.

The Salus Palliative Care team in South Bay does not require any patient to have these documents, but they do provide patients with information on the importance of them and connections to community resources to create them, if the patient desires to do so.

You make these decisions. Our job is to provide you with information and resources to help guide you in decision making, but your decisions will always be respected and followed.
