Salus Palliative Care professionals are here to help you clearly communicate with your loved ones. Our social workers will guide you through this process, help you to answer important questions for yourself before having conversations with others and support you in staying true to your wishes. Social workers can also mediate during conversations and ensure that everyone clearly understands the information you present.
The Salus Palliative Care team in South Bay does not require any patient to have these documents, but they do provide patients with information on the importance of them and connections to community resources to create them, if the patient desires to do so.
